In this opening episode of the Convergence Era, Melissa joined host Rebecca Adelman, Esq. to examine where senior living risk is actually created. Drawing on years of experience in healthcare and senior living defense, the conversation moved past surface-level risk indicators to explore the system-level breakdowns that most often generate exposure: documentation gaps, workforce strain, communication failures, and misalignment between clinical care, operations, and leadership.

A central focus of the discussion was the distinction between good care and defensible care, and how organizations can better align both through earlier intervention, stronger systems, and more intentional communication practices.

“Risk in senior living rarely escalates because of a single clinical decision,” Adelman noted. “It escalates when systems break down, when information doesn’t move, and when organizations are reacting instead of responding.”
